0254 Metastases of prostate carcinoma to the mandibular condyle

Objective: Prostate cancer is a disease that occurs in man mostly over the age of 50. Metastatic prostate cancer has a poor prognosis, with average survival rates of 1 to 3 years after diagnosis. Frequent sites for metastases are the skeletal system and lenf nodes; metastases to the condyles are rare. In this case, we report a metastatic prostate carcinoma in a 64-year-old male, who initially presented with mild pain in the right molar region. The purpose of our study was to report diagnosis, through use of a panoramic radiograph, of mandibular condylar metastases as the first sign of metastatic disease in a patient with a known primary solid malignant tumor. Methods: The panoramic radiography is the technique of choice in the general practice and provides adequate screening images of the temporomandibular joints, mandible, maxilla, teeth and surrounding tissues. Present study during the clinical and radiographical examination panoramic radiograph was taken from the patient. The panoramic radiograph revealed the presence of an osteolytic lesion within the condyle and the open-closed TMJ radiograph confirmed this. The provisional diagnosis was metastatic disease to the right mandibular condyle. Results: Routine panoramic radiograph incidentally revealed metastases of prostate ca and the open-closed TMJ radiograph confirmed this. To exclude the possible diagnosis of condylar metastases, a computed tomographic scan was obtained, which revealed that the right mandibular condyle was affected. Conclusions: Metastases to the jaws are a rare phenomenon, but about 1% of all oral cancers are metastases of primary tumors elsewhere in the body, and are located in the soft tissues as well as in the jaw bones. Therefore pain of uncertain origin in the jaws should alert clinicians to the potential of metastatic disease in patients with a history of cancer and the clinicians should take a panoramic radiograph during dental examinations.
