0248 Management of internal and external root resorption and perforation

Objectives: In this report, a case is presented where both internal and external resorptions exist in the mandibular first molar. Methods: Pathological root resorptions which can result in partial loss of dentine and cement tissues, can be classified as internal and external resorptions. In root resorptions which originate from a choronic periapical infection, two resorption types can be seen in the same tooth. Internal resorptions which are frequently encountered in the first and second molars can also occur following vital amputation with calcium hydroxide. Endodontic therapy, endodontic therapy following recalcification with calcium hydroxide, surgical intervention and extraction are possible options of treatment for the management of root resorptions. The resorption was arrested by endodontic treatment following recalcification with calcium hydroxide. The iatrogenic perforation beneath the vestibular cervical portion of the same tooth was sealed by MTA. Following endodontic treatment, the broken lingual wall extending below the gingival margin and the missing mesial and distal walls were restored with a porcelain inlay. Results: In the one year follow-up period, the resorption process had stopped and the periapical lesion had healed completely. No damage was observed in the inlay restoration. Conclusion: This is an interesting case which demonstrates the achievement of a successful result by a multidisciplinary approach.
