0465 Periapical lesions after over obturation with AH plus sealer

Objective: The aim of this study was to determine radiographically, the role of over obturation with AH Plus sealer( Dentsply/DeTrey, Konstanz,GERMANY) in the healing of permanent teeth with apical periodontitis. Material and methods: A hundred and twenty seven permanent teeth (incisors, premolars, molars ) which were clinically asymptomatic, did not respond to sensitivity testing, had not received previous endodontic treatment and showed radiographic evidence of periapical bone loss were selected for this study. The root canal treatment was complated with gutta-percha cones and AH Plus sealer by using lentulo spiral and lateral condensation techniques. After radiographs were taken, from all the root canal treatments, only 49 were detected as overextention and those were involved to this study. Healing of periapical radiolucency was evaluated radiographically 1, 2, 3, 4 years after treatment. The criteria for success was complete healing and incomplete healing. The criteria for failure was no healing. The amount of over extended material was also judged as unchanged, less, or absent at the periapex. Z Test was used to evaluate the statistical significance of results. Results: Ultimately, a total number of 49 cases were included in this study, of which 45 ( 91.83 %) were classified as successfull, and 4 ( 8.16 % ) as failure after the observation period. The difference between these groups was statistically significant (p<0.05). The amount of over extended material was classified as “unchanged” in 13 (28.88 %) cases, as “less” in 12 (26.66 %) cases and as “absent” in 20 (44.44 %) cases after the observation period. Conclusion: Extruded AH Plus sealer and the presence of preoperative periapical lesion don't have influence on the outcome of treatment in well-treated teeth. Radiographic disappearance of material extruded beyond the root will be seen by the time.
