Objectives: This study aims to evaluate the levels of the cytokines (IL-4 and IL-10) in gingival crevicular fluid (GCF) and determine the correlations between the clinical parameters and the levels of cytokines in periodontitis and RA patients. Methods: A total of 51 patients in 3 groups were included in this study (17 patients with moderate periodontitis (P), and 17 systemically and periodontally healthy patients (C) and 17 RA patients). The RA patient group was divided into two groups, the group RA-a (in sample site, pockets <3 mm, n=12) and the group RA-b (in sample site, pockets >3mm, n=5). For each patient, probing depth (PD), gingival index (GI), plaque index (PI), gingival bleeding time index (GBTI), and clinical attachment level (CAL) values were recorded. ELISA assays for quantitative detection of IL-4 and IL-10 were employed. Results: IL-4 levels have found different between the RA groups (a and b) and C group, RA-b and P groups, and P and C groups (p<0,05). The differences in IL-10 levels between the RA-a and C, RA- a and P, RA-b and C, and between P and C groups were statistically significant (p<0, 05). The correlation between IL-4 and IL-10 were significant in the RA-a and P groups (p<0,01). The RA groups, when evaluated together, have presented correlations between IL-4 and CAL. Conclusion: In RA, a specific pattern of synthesis and degradation of cytokines could occur within the gingiva, producing changes in the cytokines ratios as the disease progresses. Anti-inflammatory drugs may effect on the gingival inflammation and the synthesis of anti-inflammatory cytokines. IL-4 and IL-10 may be considered promising modulators in the control of the RA and P.
