0245 Are Hopeless Teeth Really Hopeless?

Objectives: The aim of this study was to evaluate the clinical results of intentional replantation of periodontally involved extruded hopeless teeth after applying platelet rich plasma (PRP) to artificially deepened sockets. Methods: Four extruded teeth with hopeless prognoses because of extensive alveolar bone loss and deep pockets extending between apical 1/3 of the root and apexes were selected. Inter-incisal/occlusal distance between the hopeless teeth and adjacent healthy teeth were measured. A mucogingival flap was elevated and the teeth were extracted so as not to damage the healthy remnant of the periodontal ligament remaining on the root surfaces and alveolar bone. Extra-oral debridement was performed on the contaminated root surfaces and the remaining periodontal ligament was conserved. Alveolar sockets were deepened by a drill so as to replant the teeth to their pre-extruded positions. So periodontal ligament remnants which exist on the root cementum would face to artificially prepared alveolar socket, the root planed surfaces, which had been periodontally involved, would face to surfaces of periodontal ligament remnant containing alveolar bone and the initially periodontally involved connective tissue. After applying platelet rich plasma (PRP) to socket walls and connective tissue, teeth were replanted and splinted to adjacent teeth for 2 weeks. Results: Probing depth (PD), clinical attachment loss (CAL) and radiographic examinations were performed pre-operatively, 3. and 6. months postoperatively. Mean PD was decreased from 7 to 5.5 mm and mean CAL from 8 to 6 mm and no root resorption or ancylosis was observed. All teeth were well maintained without any discomfort for the maintenance period of 6 months. Conclusions: Even during the short period of evaluation, it can be suggested that replantation of hopeless teeth using PRP can be an alternative approach to extraction in cases where advanced periodontal destruction is present and no other treatments could be considered.
