3014 BTM Mixtures Cause Majority Of Oral Carcinomas In South Asia

Objectives: Based on alarming concerns of the correlation of oral carcinomas with chewing various betel-nut and tobacco mixture (BTM) substances by South Asian males led to this preliminary clinical study to assess the form and frequency of the carcinogenic activity of these compounds.

Methods: In the first stage of this investigation, volunteer males with habit of BTM chewing in four age groups underwent a clinical study which recorded salient clinical and pathological data from these cases, taking into account all possible factors which could be responsible for presenting carcinogenesis in the oral mucosa, tongue posterolateral margins, lips and lymph nodes. A control group comprised two sub-groups, one which occasionally used BTM and the other which consisted habitual users, but showing no sign of neoplasia. Acute or chronic ulceration, leucoplakia, neoplastic transformation, growth of primary tumors, vascularization and any possible metastases were especially marked and biopsies were carried out.

Results: BTM chewing presented as the single major aetiological factor in approximately all (96%) cases of oral cancer. The majority of cases were diagnosed with squamous cell carcinoma, which in about 4% cases seen had been metastasized. Control group showed no signs of malignancy, except acute ulceration and leucoplakia in 75% of the cases.

Conclusion: Chewing of betel nut-tobbaco mixtures or derivative substances singles out as the conspicuous and major carcinogenic reason for the oral carcinomas in 96% of the cases studied, confirming the earlier concerns that these compounds are potent carcinogens. However, there is a need to study any possible genetic mechanisms (such as the role of p53 gene) underlying induction of carcinogenesis or any antagonistic effect on the spread of malignancy. It is recommended that extraordinary measures be taken on urgent basis in the region to discourage the consumption of tobacco and betel nut mixtures, especially by youngsters.
