1794 In-office Bleaching - objective interpretation of color change

INTRODUCTION: to date most clinical studies evaluating tooth whitening capabilities of bleaching products rely on the use of shade guides and the clinician's perception of color change. This perception is very complex and subjective, as often such factors as ambient lighting, the color of a patient's skin, make-up and clothing may change over the period of the study. OBJECTIVE: This pilot study first performed a subjective evaluation of color change on patient's teeth with Vita Classical Shade Guide (VCSG) and Vitapan 3-D master tooth guide (V3-DMG). Then an objective evaluation was preformed using a Spectro-photometer,Vita Easy-Shade(VES)(Vident). METHODS: A total of 15 subject's ages 18-25yrs with baseline tooth shades ranging from VCSG A1-C4 were selected. 12 patients were treated with Zoom (Z)(Discus Dental) and 3 with Boost(B) (Ultradent).Baseline(bl),post-operative(po),24hr,7day(7d), and 1 month(1m)readings were taken of all maxillary anterior teeth using VCSG, V3-DMG, and VES. The clinician performing bleaching procedure was not involved in baseline shade taking, and those evaluating shade at recalls were not aware of bleaching product used. RESULTS: Both Z and B showed dramatic color change using VES, VCSG,and V3-DMG at 24hr,7d, and 1m recalls. Immediate po, VES showed some unexpected variation in readings for each tooth treated while VCSG and V3-DMG showed stark color change for each tooth treated. At 24hr,7d, and 1m recalls VES was very consistent with its readings. CONCLUSIONS: (1)Both Z and B performed well in this study with significant and stable color change over the period of the study,(2)the bleaching process may effect accuracy of VES readings immediately po , (3) objective use of VES is more stable than subjective use of VCSG and V3-DMG at 24hr,7d, and 1m recalls.
