Local Delivery of Medications for Periodontal Therapy

Description: Following two landmark reports by Goodson and co-workers and Lindhe and co-workers in 1979, there has been intense interest and research on the use and efficacy of medications, locally delivered into the periodontal pocket, alone or in conjunction with scaling and root planing (SRP), for the management of chronic and aggressive periodontitis. Initial research examined the effect of locally delivered tetracycline, contained in non biodegradable ethyl vinyl acetate fibers, as an anti-infective medication for disease management when used alone or with SRP. The positive results from the tetracycline fiber studies quickly led to an exciting era of developing new and easier to handle biodegradable vehicles for delivering a variety of exciting antimicrobials to the periodontal pocket for disease management. Randomized controlled human trials indicate that the antibiotics metronidazole, doxycycline and minocycline and the antiseptic chlorhexidine, when locally delivered to the periodontal pocket, result in a significant reduction in pocket depth and gain in clinical attachment level. There is also intense interest in the efficacy of locally delivering bone-sparing agents to the periodontal pocket. How efficacious will host modulating medications, that block tissue destruction, be in disease management? Initial findings from human studies show much promise. In addition, recent studies in tissue engineering have shown that regenerative molecules such as growth factors can be delivered to local periodontal lesions to promote repair of periodontal structures. These novel devices have demonstrated potential for the targeting of drugs in bolus, time programmed and pulsatile fashions to optimize drug delivery and act as “biomimetic” scaffolds for tissue neogenesis. Finally, the findings from this field of research indicate that locally delivered medications can contribute clinically significant advances to dentistry for the management of a wide-spread human condition
