1058 Diagnosis and management of necrotizing ulcerative gingivitis by dental practitioners

Objectives: The aim of this study was to investigate the diagnosis and management of necrotizing ulcerative gingivitis (NUG) in general dental practice. Methods: We recruited 20 dentists to participate and they were issued with standardised assessment sheets to complete for each patient presenting to their practice with NUG. This recorded relevant clinical details and a summary of the initial therapy provided. Dentists were asked to refer NUG patients to attend our research clinic for a comprehensive periodontal examination. Diagnosis of NUG was based on the presence of papillary ulcers, pain and gingival bleeding. Results: The study resulted in 41 returns of completed data sheets from 15 dentists. Dentists identified NUG in 16 males and 25 females aged between 16 and 34 years (mean 21.8 SD 4.6). 27 patients smoked cigarettes. Ulceration was observed in all cases and the extent of ulcers ranged from 2 to 17 sites (mean 7.7 SD 3.7). 93% patients were prescribed antibiotics usually Metronidazole although dosage and length of course were variable. 76% were given a mouthwash and had scaling, 85% had oral hygiene advice and 67% of smokers were advised to quit. 18 patients agreed to be examined by a periodontist. These individuals had a mean ppd of 2.4 SD (0.5) mm and mean cal of 0.9 (0.8)mm. Seasonal variation was observed with NUG presentation most common in the summer months with 68% cases occurring from May to September. Conclusion: NUG is still a significant problem for young adults attending general dental practice and that initial diagnosis and management is appropriate.
