J. HAMILTON, J.B. DENNISON, K. STOFFERS, and W. GREGORY, University of Michigan, Ann Arbor, USA Objectives: To evaluate the clinical quality of preventive resin restorations (PRR) in a five year study placed in patients between the ages of 11 and 36. Methods: A randomized clinical trial was started to evaluate the benefits of early operative intervention into questionable carious lesions in the pits and fissures of posterior teeth (JADA December 2002, p1643-51). 223 teeth with questionable carious lesions from 93 patients were randomized into an early treatment group (ETG) and a control group. This study will evaluate the clinical quality of those restorations placed in teeth in the ETG. Each of the 113 teeth in the ETG was isolated with a rubber dam and the questionable carious lesions operatively treated with air abrasion (KCP 1000). The preparations and radiating fissures were etched with a gel etchant (Total Etch), bonding agent (Syntac SC) applied according to the manufacturer's directions, and a flowable composite (Tetric Flow, shade A2) was placed in the preparation and radiating fissures and then light cured. Two dentists evaluated the restorations independently at six month recalls for up to 60 months utilizing a modified Ryge Criteria to evaluate color match, marginal discoloration, marginal adaptation, anatomic form, and surface smoothness. Results: The following are the percent alpha evaluations for the modified Ryge Criteria during the five year clinical trial: Color Match 98%, Marginal Discoloration 92%, Marginal Adaptation 73%, Anatomic Form 95% and Surface Smoothness 93%. Only four restorations required repair, and those were all due to penetrating stain. Conclusion: The preventive resin restorations performed very well in this five year clinical trial. This study was partially supported by Delta Dental Fund of Michigan. | ||
