MERICSKE-STERN, University of Bern, School of Dental Medicine, Switzerland Objectives: Sophisticated denture designs, i.e. perio-overdentures with goldcopings and attachments have evolved from simple overdenture treatment modalities. Some limitations were observed due to asymmetrical distribution of the remaining tooth-roots. Additional support can be achieved by placement of single implants. Results with removable partial prostheses supported by roots in combination with single implants were not yet available. The purpose of the present study is to report on preliminary results. Methods: Among wearers of perio-overdentures 15 patients (mean age 66.5) were identified who had additional implant support, i.e. 4 implants in the mandible and 13 in the maxilla. The patients had regularly attended the recall during a mean observation time of 2.3 years. In the context of the present study they underwent a clinical examination with records of periodontal / periimplant parameters and single radiographs were obtained. The patients' response to the treatment was assessed by 9 evaluation criteria (VAS). A data collection of all technical problems was available. Results: A 100% survival rate of the implants was found, whereas 2 roots were lost. The periodontal / periimplant parameters showed low plaque and bleeding indeces. 7 tooth- and 2 implant-sites exhibited PD over 4mm. 2 implants were found with more than 1 mm crestal bone loss. Prosthetic complications were mostly related to the female retainers of retentive attachments (implants and root-copings) and 6 copings had to be recemented. The mean values of the VAS-evaluation ranged between 70 and 93%. Conclusion: These still limited observations demonstrate that single implants can be successfully used for overdenture support together with tooth-roots. Professional maintenance and prothetic service are regularly required. | ||
