1051 Laminin-5 and Collagenases in GCF of Insulin-Dependent Diabetics

The tissue destructive host proteinases including matrix metalloproteinases (MMPs) can degrade extracellular matrix and basement membrane components, including laminin-5 (Ln-5) of epithelial/endothelial basement membrane component in gingival tissue. Objectives: To characterize the levels and forms of the ã2-chain of Ln-5 and MMP-1, -8 and -13 in poorly controlled insulin-dependent diabetic (PIDD) and controlled insulin-dependent diabetic (CIDD) gingival crevicular fluid (GCF) and compared they to that in healthy control respectively. Methods: GCF samples (PIDD, n=18; CIDD, n=5, controls, n=6) were analyzed for the expression of Ln-5 fragments and forms of collagenases by using Western immunoblotting methods. The activity of collagenases was evaluated electrophoretically using type I collagen as substrate and analyzed dentisomertically. Results: Ln-5 fragments and pro- and active forms of MMP-1, -8 and –13 were clearly detected in GCF of PIDD subjects, whereas in GCF of CIDD and control subjects, faint, if any, Ln-5 fragments and different molecular forms of collagenases could be detected. The activity of collagenases in GCF samples was significantly higher in PIDD compared to control group (p< 0.01), whereas the difference between CIDD and control group was not remarkable. Conclusion: These findings suggest that GCF of PIDD is characterized by elevated level of Ln-5 fragments and collagenases and thus, these biochemical GCF findings confirm and further extend our previous clinical study overall evidencing that PIDD subjects have an increased risk for periodontal disease than CIDD subjects.
