2460 The Sensitivity and Specificity of Miniature Bruxism Detection Device

Objectives: The gold standard for bruxism detection is a polysomnogram but this method is not easily applied to large clinical studies. One solution is a miniature self-contained EMG detector-analyzer (BiteStrip®, S.L.P. Israel) that can be applied directly to the masseter muscle. It records the number of masseter activation above a preset threshold. The validity and reliability of this system needs evaluation. Our study determined the sensitivity and specificity of BiteStrip recording versus masseter EMG recordings during a polysomnogram. Methods: Five healthy volunteers (1 female and 4 male, mean age 31.6+/-3.05) who had suspected nocturnal bruxism participated. Each subject underwent a standard overnight polysomnographic study that included masseter electromyography (EMG) concomitantly with a BiteStrip recording. These data were compared and the sensitivity and specificity were estimated applying the several cut-off EMG thresholds (20%, 30% and 40% of maximum voluntary clenching). Results: The mean EMG-detected bruxism events above threshold were 1.71+/-0.77, 2.67+/-1.09 and 5.68+/-1.19 per hour (40%, 30% and 20% MVC, respectively). All subjects together produced a low sensitivity and a moderately good specificity. It was apparent that subjects that exhibited few EMG events had the lowest sensitivity. In fact, the coefficient of correlation between sensitivity and number of EMG events were 0.75, 0.82 and -.29 (40%, 30% and 20% MVC respectively). In contrast, the specificity was largely unaffected by the number of bruxism events (range = 0.55 to 1.00). It was determined that using the 40% MVC threshold and excluding subjects that exhibited fewer than 15 BiteStrip events, produced the best combination of sensitivity (0.63) and specificity (0.73). Conclusion: This device has good specificity for all subjects but fair sensitivity for subjects that exhibit moderate to high levels of EMG determined bruxism. The device appears to be a cost effective tool to screen for moderate and high-level bruxism subjects.
