QIAN1, 1University of Iowa, Iowa City, USA, 2Wake Forrest University, Winston-Salem, NC, USA Objectives: The American College of Prosthodontists (ACP) published a classification system for complete edentulism in 1999. This study evaluated the ACP Classification system which was supplemented with a patient satisfaction questionnaire survey, the OHIP-14, and a Depression Inventory. The hypothesis of this study was that patients identified as ACP Class 3 or 4 would be less satisfied with their dentures but that the classification could not specify whether that dissatisfaction was related to esthetics, comfort, and/or function. Methods: 100 patients who had complete maxillary and mandibular dentures made in the predoctoral clinics at the University of Iowa between 1996 and 2002 were recalled. The subjects and their dentures were clinically examined, and a panoramic radiograph was made so as to classify these patients according to the ACP system. Subjects completed several self-administered questionnaires; the Oral Health Impact Profile - 14 (OHIP-14), the Geriatric Depression Scale, and a denture satisfaction questionnaire. Descriptive and bivariate analyses were performed by Chi-Square test or Fisher's exact test if the sample size was too small. Results: Subjects who were classified as ACP Class 1 and 2 were combined and compared to ACP Class 3 and 4. The data provided evidences of a significant association between the ACP Classification Systems and patient satisfaction with comfort of their dentures (p= 0.0052). No significant associations were found between the ACP Classification System and patient satisfaction with denture esthetics (p= 0.2753), nor with denture satisfaction with function (p= 0.1123). Conclusion: Patients classified as ACP Class 3 or 4 were more likely to be dissatisfied with their dentures. However, additional question items are needed to clarify the nature of their dissatisfaction. ACP Class 3 and 4 patients in this study were dissatisfied because of discomfort not problems with function or esthetics. Funded by the ACP/ESPE Fellowship in Geriatric Prosthodontics.
