| 1619 Immediately Loaded Implants in Edentulous Mandibles: 4-year Experience | ||

STEVELING, Zentrum fur Zahn Mund und Kieferheilkunde, Heidelberg, Germany Objectives: Examitation of immediate loaded dental implants in the mandible after 4 years function. Materials and methods:20 Patients with edentulous mandibles at the average age of 68,5 years were treated with 4 – 6 ASTRA-Universalimplants. 96 ASTRA-Universalimplants were placed without any antibiotic prophylaxis. The distal implants were placed with a security distance of 3 mm from the foramen mentale. The residual implants were inserted in between the section of the distal implants. The prosthetic construction was placed on 20 degrees universalabutments within 3 days after the operation. The overdenture showed a distance of 2 –3 mm to the soft tissue. The metal framework was completely covered with acrylic material in order to create a uniform distance between the mucous membrane and the tooth replacement after the final healing of the soft tissue.The prefabricated acrylic teeth 36 - 46 were fixed with Palapress Vario, after cephalic articulation of the models on the metal framework. All 20 patients were previously provided with a new complete denture prosthetic in the maxilla. Clinical controls were made 10 days after the operation, after 6 and 12 weeks, and finally after 6 months. X-ray controls were made after placement of the fixed restoration after 6, 12, 24 und 36 months.Results:After 6 months one implant failed. It was removed and not replaced. All other implants revealed non-irritated soft tissues and are still in function. The average boneloss was 0,6 mm (min. 0,0 mm, max. 1,3 mm) after 4 years. All patients were satisfied with the results. Conclusion:The use of a single-stage surgical technique with ASTRA-Universalimplants with immediate loading of fixed restorations has proved to be successful and economical in edentulous mandibles. As good oral hygiene can be maintained easily, this method is recommendable especially to older patients. | ||
