2021 Evaluation of a Titration Test for Use of an Oral Appliance for Obstructive Sleep Apnea Syndrome Therapy

Objectives: Therapy by oral appliance (Sleep-Splint) is well known as one of the conservative treatment for Obstructive Sleep Apnea Syndrome (OSAS). When mandible position is advanced by Sleep Splint (SS), air-way-space will be spread widely. It is the most important for making SS to decide the amount of mandible advancement. The titration test is one of the diagnostic method to estimate the appropriate amount of mandible advancement. But it is not almost carried out before SS is made in Japan. The purpose of this study was to evaluate the usefulness of the titration test with test splint. Methods: Before SS was made on a semiadjustable articulator, the titration test was carried out to ten adult OSAS or Habitual Snoring patients. All patients were laid in turning over and the test splint was fixed in each mouth. The place where the ventilation of the upper respiratory tract improved of the patient sense was decided were the amount of mandible advancement. Results: There was the statistically significant improvement in all patients, on the AHI and SpO2 change after the new SS made on the titration test was set. Though the side effects such as discomfort of TMJ or several teeth were often observed immediately after wearing the SS, all cases were disappeared in the short periods. Conclusions: These results suggest that the titration test is useful diagnostic method of SS therapy for OSAS. In the future this test can be applied to the polisomnography.
