| 2847 A Complete Review of Papillon-Lefèvre Syndrome: A Case Report | ||

M. MIR, Beheshti Center of Dental Research, Tehran, Iran, and G.H. RAMEZANI, Shahed Dental School, Tehran, Iran Objectives: Introducing a new treatment method for Papillon-Lefevre Syndrome. Methods:Different examinations performed as follow: Case History:A 3.5 year old girl patient presented with 10 missing and 6 mobile primary teeth in April 1998. Physical examinations revealed palmar and plantar hyperkeratosis. No other physical, mental or laboratory disorder was found. Clinical Feature:Dental examinations showed severe generalized gingival attachment loss in both dental arches.There was a root exposure around all present teeth. Radiographic Evaluation:Severe bone loss was evident in panoramic and occlusal radiographs. Diagnosis and Treatment:This rare autosomal recessive syndrome was treated with daily chlorhexidine mouth rinse. All primary teeth was extracted in June 1998 to eliminate source of infection and prevent tooth aspiration. Microbiology:The early antibiogram detection showed cephalexine as the antibiotic of choice for the disease. The result of cultures revealed the predominant presence of Bacterioids. Histopathologhy:Hypercementosis and inflamatory reactive (fibrosis) hyperplasia was observed in the slides of the involved teeth and surronding tissues, respectively. Results:Early antibiogram and finding the best antibiotic for recommending after extraction of teeth prevents the furture infection and need to antibiotic therapy before the eruption of permanent teeth. Follow-up:Infection was successfully controled. The patient was reevaluated clinically and paraclinically and no future antibiotic therapy was needed. Permanent incisors and first molars has been erupting under good oral hyegine care according to the last recall(November 2002). No significant finding was found in her recent panoramic X-ray. Conclusions:Actual microbiological tests will conclude the best treatment. | ||
