| 3161 Prosthetic complications in an implant overdenture population at 36 months | ||

MORIARTY, University of North Carolina School of Dentistry, Chapel Hill, USA Objectives: One purpose of this on-going five year prospective trial was to assess the clinical performance, and prosthetic complications of mandibular overdentures retained by two Microthreaded implants placed in the canine regions using gold cap attachments. Methods: Fifty-six completely edentulous patients were restored with new complete dentures. Two Microthreaded implants were placed in the #22 and #27 regions in a one-stage surgical approach. Three months later, solid ball abutments were placed, and the denture retrofitted with gold cap attachments (baseline). Radiographic (tomograms), periodontal, and prosthetic parameters were measured at baseline, and at 3, 6, 12, and 36 month recall intervals. Results: Of 56 initial patients, three have been discontinued from the trial. A total of 372 complications were noted for 51 patients. Seven patients (12.5%) had no complications related to the mandibular prosthesis or implants. In descending order of occurrence, tightening of the gold cap attachments (101 occurrences in 42 patients, mean of 1.8 +/- 1.8 per patient, range 1-11) > adjustment of denture sore spots (44 occurrences in 22 patients, mean of 0.8 +/- 0.8 per patient, range 0-5 adjustments) > relining of mandibular denture (23 occurrences) > replacement of cap attachments due to loss of retention/wear (23 occurrences) > repair of broken or cracked dentures (19 occurrences) > relining of maxillary denture (15 occurrences) > replacement of broken/debonded denture tooth (14 occurrences) > replacement of worn ball abutment (13 occurrences). The mean interval to the first complication was 382 +/- 356 days (range 13-1,126). SEM photomicrographs were taken of the worn titanium ball abutments for analysis. Conclusions: Although a proven safe and effective treatment modality, implant retained overdenture prostheses can and do require periodic maintenance. The majority of complications in this study were prosthetic, rather than implant/abutment, related. | ||
